Thank you for entrusting ABC Veterinary Hospital Pacific Beach with your pet travel needs. Our skilled and experienced veterinarians are USDA-accredited for domestic and international pet travel. We have proudly assisted over 1,000 pets in reaching their travel destinations!

Please carefully read and fill out the important information below. The information provided within the Health Certificate Intake Form below will be entered into your pet's official health certificate and submitted to the US Department of Agriculture. Please take time to ensure the information is complete and accurate. Please be sure to provide all the requested information, as this will minimize the back-and-forth needed. ABC Veterinary Hospital Pacific Beach is not liable for incurred fees and/or declined entry into destinations due to errors within the Health Certificate Intake Form.

Please note that completion of this form does not secure your appointment booking. After pressing "Submit", your form will be automatically sent to our Pet Travel Specialist. Once received, you will be contacted by our Pet Travel Specialist to discuss any additional items needed and/or to book your appointment.

For all international destinations, we require a minimum of 30 days' notice prior to travel. All international health certificate appointments are subject to approval by our Pet Travel Specialist prior to booking. Note: If you require urgent assistance for immediate upcoming travel within 30 days or less, please complete the form below and contact us at (858) 270-4120. Please note that your travel request may not be accommodated due to timeline restrictions and depending on your pet’s medical records.

In no event shall ABC Veterinary Hospital Pacific Beach be liable for financial loss(es) incurred as a result of pet travel, including but not limited to: errors in Health Certificate information, travel delays, flight re-bookings/cancellations, quarantine fees, hotel/accommodation costs, etc.

Please note: Pet travel services with ABC Veterinary Hospital Pacific Beach include exportation from the US ONLY. ABC Veterinary Hospital Pacific Beach does not assist with the importation or return of pets to the US. Requirements for returning to the US are affected by the length of time outside of the country, layovers, and other specifics related to your destination country. Returning to the US is the responsibility of the pet owner. For guidance on returning to the US, please contact the USDA, CDC, or a local veterinarian at your destination for assistance. Lengths of travel, layovers, and destinations may vary with their requirements for return.*

If flying into a country/airport that is not your final destination, please list your “plan” (ie. drive right from airport to final destination same day, stay the night and then drive to destination) this could affect the requirements for your pet’s travel.

*Your pet will be scanned for a microchip during your visit, if your pet does not have an ISO compliant microchip, we strongly recommend (may require) one, as the CDC recently changed regulations for re-entry to the US requiring an ISO compliant microchip.

Rabies Vaccination History

If your pet did not receive a Rabies Vaccination at our facility, please email the original copies of the rabies vaccination certificates.

Please note: The Rabies Certificate must include: your pet’s microchip number, administering veterinarian's signature, and all of the rabies vaccine information (lot #, manufacturer, vaccine name, vial expiration).

Please email BOTH Rabies Certificates (if applicable) AND your pet's previous records to info@abcvets.com.

Please note that if your pet is not up to date with a rabies vaccine, and/or does not meet travel requirements for your desired destination, you may be advised to postpone your travel date.
