Our skilled and experienced Veterinarians are USDA accredited for domestic and international pet travel, and always have the most up to date information with regard to USDA and CDC regulations. We have proudly assisted over 1,000 pets reach their travel destinations! Please carefully read and fill out the important information below, this is not an official intake; this is an inquiry submission to get started. Please be sure to provide all of the information that is asked, as this will minimize the back and forth needed.

Please note, completion of this form does not secure your appointment booking, or starting the travel process. After pressing "Submit", your form will be automatically sent to our Pet Travel Specialist who will review the travel assistance request, upon approval we will reach out to you to get officially started on the process.

For all international destinations, we require a minimum of 30 days' notice prior to travel. All international health certificate appointments are subject to approval by our Pet Travel Specialist prior to booking. Note: If you require urgent assistance for immediate upcoming travel within 30 days or less, please complete the form below and contact us at 858-270-4120.

Please note, your travel request may not be accommodated due to timeline restrictions, and depending on your pet’s medical records. If your pet is 10+ years old, we may require a physical exam to ensure your pet is healthy enough for travel. This is an additional requirement and is an additional exam fee.
